Conflicts Disclosure Information and Forms

Chapter 176 of the Texas Local Government Code may require the public disclosure of certain information concerning persons doing business or seeking to do business with DART, including affiliations and business and financial relationships such persons may have with local government officers.

A copy of the Conflict of Interest Questionnaire for vendors or other persons doing business with DART is available in PDF here. The completed Conflict of Interest Questionnaire should be mailed to:
